Dementia Support Workers (DSW)
Our Dementia Support Workers have been trained with specialist skills and knowledge to support people living with dementia. Dementia Support Workers provide one to one individualised support with the aim of prolonging independence and self-esteem for the person with dementia. The main aim is to support the person in maintaining their ability. For example they can help you organise your daily schedule and support you with tasks you may be having difficulty with such as doing your shopping. Another objective is to support you in remaining socially connected and participating in activities you enjoy e.g. walking, swimming, going for a coffee, joining a local group. The support workers are insured so they can bring people out in their cares.The service may also involve support to engage with activities in the home e.g. art, crafts, computer, reminiscing, gardening, baking etc.
How do I avail of this service? If you have been referred to the Living Well with Dementia Service, the dementia nurse specialist will contact you/family member to organise a home visit and assess whether this service would meet your needs. If you haven't already been referred to the Living Well with dementia Service by your doctor or public health you or a family member can contact the nurse at 087 0550050. |
