Post Diagnostic Support
For people newly diagnosed with dementia, post diagnostic support can be provided on a one to one basis, following referral by the memory clinic, your GP or PHN. The nurse specialist will contact you to offer you an appointment to meet with her in the Memory Technology Library which is in the Grounds of South Tipperary General Hospital or where appropriate she will do a home visit.
The visit allows you and your family to discuss the diagnosis and any queries you may have. It also allows an opportunity to explore ways that will assist you in living as well as you can with dementia.
People referred with 'mild dementia' can avail of a memory rehabilitation programme. This is a group programme which is run 4-5 times a year over five weeks and provides people with practical information, strategies and techniques to assist them with day to day activities.
For anyone else diagnosed with dementia, support and information can be arranged by contacting Peggy or Anne, Dementia Nurse Specialists on 087 0550050.
