For people newly diagnosed with dementia, post diagnostic support can be provided on a one to one basis, following referral by the memory clinic, your GP or PHN. The nurse specialist will contact you to offer you an appointment to meet with her in the Memory Technology Library which is in the Grounds of South Tipperary General Hospital or where appropriate she will do a home visit. The visit allows you and your family to discuss the diagnosis and any queries you may have. It also allows an opportunity to explore ways that will assist you in living as well as you can with dementia. We also offer two group programmes: Programme 1 is a six week memory rehabilitation programme for people diagnosed with 'mild' dementia that provides them with practical information, strategies and techniques to assist them with day to day activities. It also includes information about dementia, how the memory works, treatment, living well and legal issues. It runs as follows: Week 1: Home visit, Week 2-5: Group programme held in the Memory Technology Library over 4 weeks. Week 6: Final follow up home visit. Programme 2 is a five week group programme available for people who have mild to moderate stage dementia. |
